All 29 of Kaiser Permanente Mid-Atlantic Medical Centers Recognized by NCQA for Patient-Centered Medical Home Model

August 7, 2015 – Kaiser Permanente of the Mid-Atlantic States announced today that its Mid-Atlantic Permanente Medical Group‘s (MAPMG) primary care practices received the National Committee on Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Level III rating at all 29 of its medical centers. The PCMH Level III rating is NCQA’s highest recognition for integrated and coordinated care delivery. Kaiser Permanente scored high assessments for: patient centered access; team based care; population health management; care management and support; care coordination; and performance measurement and quality improvement.

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that the Mid-Atlantic Permanente Medical Group has the tools, systems and resources to provide its patients with the right care, at the right time.”

As the first health organization in the region to utilize the PCMH model widely across its membership base, Kaiser Permanente is working to combat many of the most common concerns in American health care nationwide — runaway costs, redundancy and waste, lack of access and insufficient information sharing.

“The Permanente model of care is often referenced as an originator of patient-centered care concepts,” says Bernadette Loftus, MD, associate executive director for The Permanente Medical Group.

Since 2009, through the rigorous application of PCMH concepts, Kaiser Permanente has achieved multiple national No. 1 quality-measure ratings and, reduced emergency room visits and hospital day rates by more than 20 percent, while increasing patient satisfaction scores.
